From Referral to admission
Referral Documentation
When we receive your referral and required documentation for admission, we work with you and your caregiver to send the appropriate care team to your home to complete a comprehensive assessment and individualized plan of care that will meet your needs.
Admission
Acceptance for services is based on your willingness and ability to function in a non-institutional environment and to participate in the care being provided. Admission is never based on your race, color, creed, sex, age, handicap (mental or physical), communicable disease(s), veteran status/history, or place of national origin.
Ongoing Care
Your Eden Health care teams are professionally trained and experienced. They will be responsible for coordinating your care and communicating with your physician (if applicable) and/or family caregivers. All care is provided according to physician orders where required, and in compliance with appropriate practice acts.
