Home Health provides short-term, physician-directed medical care in the comfort of your home to help you prevent or recover from an illness, injury, or hospital stay. The purpose of home health is to rehabilitate, stabilize, and return you to optimal health and independence.
Our agencies are full-service, Medicare-Certified offering skilled post-acute care services. We are a compassionate, cost-effective alternative to inpatient care, and will provide you with education and tools to help you reach your maximum health potential.

Included Services
Skilled nursing care is provided by a Registered Nurse (RN) or a Licensed Practical/Vocational Nurse (LPN/LVN) to manage, observe, and evaluate care. It is often needed when an individual is recovering from injury or illness. Our nurses have significant training and experience in treating and educating patients within the home setting.

Physical Therapy helps you return to your prior level of function following an illness or injury, to improve your balance and strength. Your Physical Therapists will help empower you to prevent ongoing loss of independence by developing a program to keep you active and improve your physical abilities.
Occupational Therapy focuses on improving your Activities of Daily Living (ADLs), such as getting dressed, showering or preparing for a meal by helping you adapt to your environment, modify tasks, and use equipment to improve participation in every aspect of your daily life. Our Occupational Therapists will help you manage fatigue so as to conserve energy and reduce stress.
Speech-Language Pathologists (aka Speech Therapists) help you in evaluating the treatment of breathing, swallowing, speaking, cognition and thinking skills associated with communication disorders.
Medical Social Workers are available to help discover community resources and/or assistance that you may require short-term and long-term while recovering from an illness or injury.
Home Health Aides assist you with your daily activities until you are ready to be independent. Our Home Health Aides will coordinate with therapists to assist in exercises developed within your plan of care, as well as personal care, bathing/showering and grooming needs.
Introducing Collaborative Symptom Management with Chronic Disease Management
Eden Home Health will work with you and your own personal provider to assist you with recommendations on Chronic Disease states. Your team may include home health RNs, therapists, and an MSW to assist with recommendations on lifestyle modification and management. This program can also assist with End-of-Life conversations and can provide an easy transition to hospice with hospice nurse expertise and next step consultations.
