Colette Kirchhoff MD, FAAFP is Board Certified in Family Medicine & Hospice Palliative Medicine. For 23 years she maintained an active practice in family medicine in Bozeman Montana, and for 16 years she has served as medical director of Hospice. Dr. Kirchhoff is Clinical Associate Professor of Family Medicine at the University of Washington and the FCM Director of WWAMI Medical School in Montana. She is the Planetary Health Co-Director of the University of Washington School of Medicine and helped design and implement a culinary medicine and resiliency curriculum for WWAMI. She is actively involved in physician health and served on the Physician Health Committee for fourteen years. Additionally, Kirchhoff has presented lectures on death and dying, medical aid in dying, bias, resilience, mental health, and environmental health, not only at regional and statewide conferences but also to hospice staff, the community, medical students, and colleagues. Dr. Kirchhoff lives in Bozeman with her husband Steve of 41 years. She has three children and three grandchildren.

Introducing Collaborative Symptom Management with Chronic Disease Management
Eden Home Health will work with you and your own personal provider to assist you with recommendations on Chronic Disease states. Your team may include home health RNs, therapists, and an MSW to assist with recommendations on lifestyle modification and management. This program can also assist with End-of-Life conversations and can provide an easy transition to hospice with hospice nurse expertise and next step consultations.This program might be for you if:
- You are struggling with a life limiting condition or illness.
- You are having more frequent episodes of poorly controlled symptoms.
- You are overwhelmed with multiple providers, medications, and treatments.
- You and/or your loved ones are experiencing stress related to your disease progression.
