What to Expect When you are Referred to Eden Home Health Services
1. Referral
When we receive your referral and required documentation for admission, we work with you and your caregiver to send the appropriate clinical team to your place of residence to complete a comprehensive assessment and individualized plan of care that will meet your needs. Our clinical team will work closely with you, your caregiver, and your physician to make sure your goals are met.
2. Your Care Team
Eden Home Health clinicians are professionally trained and experienced. They will be responsible for coordinating your care and communicating with your physician. All care is provided according to physician orders and in compliance with appropriate practice acts.
3. Getting Started
Acceptance for services is based on your willingness and ability to function in a non-institutional environment, and the willingness, ability, and availability of your caregiver to participate in your care.
Admission to Eden Home Health is not based on your race, color, creed, sex, age, handicap (mental or physical), communicable disease(s), veteran status/history, or place of national origin.
4. Medicare
Medicare requires that you see your primary care physician 90 days prior to admission or 30 days after to certify your need for Home Health services.

With Eden Health, you can be sure that your care professionals are some of the most competent and passionate in their field. We will work with you and your doctors to achieve your health and wellness goals.
Registered Nurses (RNs)
RNs provide direct patient care, including assessments, medication management, wound care, and chronic disease management. They educate patients and caregivers and coordinate care between physicians. RNs also recognize and respond to emergencies, helping to prevent hospital readmissions.
Licensed Practical/Vocational Nurse (LPN/LVN)
LPNs or LVNs provide hands-on patient care under the supervision of an RN or physician. Their duties include monitoring vital signs, administering medications, wound care, catheter management, and assisting with daily activities. They educate patients and caregivers, document care, and report changes in the patient's condition to the healthcare team.
Certified Nursing Assistants (CNAs)
CNAs are available on an intermittent basis. Eden Home Health CNAs provide respectful and compassionate care. CNA visits encompass personal care, light housekeeping, and brief respite. Eden Home Health CNAs are licensed in the state of which they practice and are supervised by an RN. CNA visit days and times may vary to meet your needs.
Physical Therapist
PTs help you return to your prior level of function following an illness or injury to improve your balance and strength. They will help empower and prevent ongoing loss of independence by developing a program to keep you active and improve your physical abilities.
Occupational Therapist (OT)
OTs focus on improving your Activities of Daily Living (ADLs), such as getting dressed, showering, or preparing for a meal by helping you adapt to your environment, modify tasks, and use equipment to improve participation in every aspect of daily life. Eden Home Health's OTs will help manage your fatigue to conserve energy and reduce stress.
Speech-Language Pathologist (SLP)
SLPs (aka Speech Therapists) help evaluate the treatment of breathing, swallowing, speaking, cognition, and thinking skills associated with communication disorders.
Medical Social Worker (MSW)
MSWs are available to help you discover community resources and/or assistance that you may require short-term and long-term while recovering from an illness or injury.
Home Health Aides
These Aides help you with your daily activities until you are ready to be independent. Our Home Health Aides will coordinate with therapists to assist in exercises developed within your plan of care, as well as personal care, bathing/showering and grooming needs.
Bringing Connected Care Home
