Hospice is...
Hospice services provide patients and families support and comfort as patients near the end of life. These patients are no longer seeking curative treatment, expected not to recover from the disease, and have been diagnosed with six months or less to live. We:
- Provide pain and symptom management.
- Provide medications, equipment, and supplies related to the terminal illness.
- Coordinate with physicians to decrease the need for 911 calls and hospitalizations.
- Provide four (4) levels of care based on patient needs: Routine Home Care, General Inpatient Care, Continuous Crisis Care, Inpatient Respite Care.
- Assist with personal care (bathing, shaving, etc.).
- Help decrease anxiety, depression, and other common emotions associated with terminal illness.
- Offer psychosocial, emotional, and spiritual support to patient and family.
- Focus on comfort.
Palliative Care is...
Palliative care reduces the strain of those living with serious or life-limiting illnesses that may cause increased hospitalizations and emergency room visits. You can receive palliative care any time throughout a serious illness, from the beginning of your diagnosis, on. It can also be paired with a curative treatment, including home health. We provide:
- Pain and symptom management.
- Medication and disease process education for patients and their family.
- 24/7 support.
- Emotional and spiritual support.
- Advance Directive planning.
- Assistance with community resources.
- Bereavement services and support groups
