Fast-Track to Outpatient
4-6 sessions per week tailored to patient's needs that will address immediate post-op recovery in the home while patients are struggling to safely access the community.
Good for patients with strong social support that can get to outpatient therapy following their 2-week post-op appointment.
Traditional Home Health Track
2-3 sessions per week to address patient's immediate post op recovery and sub-acute rehabilitation over the course of 3+ weeks until patients are able to gain access to community or remain safe in their home.
Good for patients who may or may not have strong social support but are cognitively intact enough to follow home exercise programs between sessions and leaving the home is taxing for them.
Pre/Post Op Home Health Track
1-2 weeks prior to scheduled outpatient surgery, PT and OT will provide patient with:
- Home preparation strategies
- Managing ADL's/Mobility with their anticipated precautions
This is a great option for those undergoing first time joint replacements to return home with confidence following surgery. Eden Home Health will then be ready to start rehab the following business day with either the Fast-Track to Outpatient or Traditional Home Health Track.

Manages surgical incision medication and disease education to help keep you safe in the home.
Home Health Aide
Assists with sponge baths or showers until you are able to safely do so on your own.
Speech Therapy
May be appropriate for those who have trouble swallowing or with cognitive skills following surgery.
Social Services
Helps access community resources, meals, transportation, review advanced directives and psychosocial adjustment.

Medicare covers home health services without any out-of-pocket expenses for your patient. Most Med Advantage plans are covered at 100% and most Commercial Insurance Plans cover Home Health services at little to no cost to their beneficiary.
